The Pre-Application Screening Form helps us review your information to determine if you may be eligible for WIOA program services. Completing and submitting the form does not guarantee eligibility. It allows the CDC Eligibility Coordinators to decide if you should be scheduled for a final eligibility determination and enrollment at your local American Job Center.

Please review the WIOA Applicant Eligibility Verification Checklist. This is the official document used by staff during the eligibility review. Only upload documents that apply to you. If you are unable to obtain a specific document, contact your local American Job Center to discuss alternatives.
Meeting eligibility requirements does not automatically guarantee enrollment. After eligibility is determined, you must also complete the enrollment process and be found suitable for WIOA services. WIOA is not an entitlement program. All applicants must complete assessments and evaluations, including a basic math and reading test. You may re-test after six months.
By submitting the pre-application form, you agree to attend all scheduled assessments, evaluations, and counseling sessions with a CDC Career Specialist to establish your needs, goals, and next steps.
